You will most probably be asked by your university to provide
immunization certificates. The health documents required vary according to the state you will be in. Your university may have a specialized form for this, so use
their form. Also, you may be required to undergo medical check up once
you are in US. Many of us will be reported as positive for many kinds
of tests, because Indians usually have more germs than people in USA.
Do not worry too much about it, they may carry out a few further tests
on you before they declare you fit :-)

Get a complete medical checkup done before you leave for USA.

Be
sure about your teeth. Student medical insurence usually does not cover many dental problems, though some universities offer a minimal coverage. Check their website for details, but it will definitely be not exhaustive. So get them examined by a good doctor before you
leave. Dental treatment is EXTREMELY costly in USA. It is cheaper to come over here, get a teeth removed and fly back rather than getting it removed in USA. and I am being perfectly serious.

The rest should be cool enough - people with specs may want to get their eyes checked up and carry extra glasses or frames. Because of weather and food and water, even if you have a poor physical constitution, you have less chance of falling ill in US than in India.
